Counselling: Free confidential general counseling for all ages; if you have a question for a Social Worker call 519-253-8481 ext. 236 Medical: If you have a question for a Nurse Practitioner call 519-997-2824 ext. 416 Health Promotion: To learn more about available health care services call 519-997-2827 ext. 339 * This site offers primary care to individuals who do not have a doctor or a nurse practitioner. No referral is required. We welcome walk-in clients. We Offer the Following Services: * Primary Care * Medical Renewal * Pediatric Care * Teen Health Services * Women's Health * Wellness Education * Chronic Disease Management * Nutrition Counselling * Social Work Counselling * Speech Language Pathology * Smoking Cessation There are several group programs offered at this site: * Diabetes Education * Active Aging Therapeutic Exercises * Master Your Health Self-Management * Chronic Disease Management Program |
