| Description (Service) | Formerly the South West Community Care Access Centre (CCAC) Health care coordination service * coordinates a wide range of in-home services to eligible clients * care coordination, nursing, physiotherapy, occupational therapy, social work, personal support * supportive care team for palliative and cancer care * elder abuse * nutritional counseling, speech language pathology and medical supplies and equipment * provides placement in long-term care facilities and respite care * facilitate application to residential hospice * assessments for government funded day away programs and supportive housing services * home and school health services for children with special needs * Care Coordinators determine eligibility for drug coverage under the Ontario Drug Benefit (ODB) Plan |
