| Description (Service) | Palliative, respite and transitional health care services for those with HIV/AIDS/HCV in a home-like environment * services include case planning, social and recreational support, nutritional services, house keeping, and community coordination Clients are admitted for a maximum of 12 months * short-term and respite stays available |
