Description and Service Details
Description (Service) Health centre focused on individual, family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * community development, health promotion and prevention services * referrals to other local and social health services * services aimed at people living with a low income, newcomers, individuals and families with chronic and complex illness

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health

Community Development - Initiatives and advocacy to build stronger and more resilient communities * LIHC works with community members and other organizations work together to take collective action and generate solutions on common issues * addresses social determinants of health by supporting individuals, families and communities

Daily Programs for Everyone - Daily programming, including art activities, health and wellness, education, and training * monthly schedule available on website

Health Promotion Workshops - Workshops and education on common health issues and health promotion strategies * focuses on prevention and self management techniques

Immigrants and Newcomers Programs - Programs and services for people newly arriving in Canada

Living a Healthy Life with Chronic Conditions - Programs and services help people with living with chronic conditions * educational workshops, one to one counseling and group sessions which support people to better manage their conditions

Mental Health Services - One-on-one, couple, family, and group counselling for registered clients * psychology and psychiatry services for primary health care clients registered with a doctor or nurse practitioner * mental health screening and assessments * short-term and long-term counselling * all services are confidential

People Experiencing or At Risk of Homelessness - Health, social services and daily programming for people who are experiencing or at risk of homelessness * nursing care * addictions assessments and referrals to treatment * harm reduction education and support * housing loss prevention * ID replacement * laundry and showers * social assistance applications and advocacy

Physiotherapy - 8-week physiotherapy program * physiotherapists assess and treats conditions that affect the muscles, nerves, and bones

Primary Care Services - Primary health care is provided for registered clients by an interdisciplinary team of doctors, nurses, foot care providers, registered practical nurses, nurse practitioners, social workers, and dietitians

Seniors Programs - Programs and services for adults aged 55 and older

Transgender Communities Health Services - Programs and services for people exploring their gender identity * one-to-one support * group sessions * general information and education for community organizations
