Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
National health care organization providing a broad range of more than 50 different programs and services, aimed at improving the health and quality of life of older adults and adults with disabilities
Family Supports Program - Support and respite to people with disabilities and their families
Homecare and Professional Services - Community health, support and in-home services by nurses and personal support workers
Passport Program - Community participation supports and respite supports for adults with intellectual disabilities * plans for support based on each person's needs and goals
