| Description (Service) | * A supervised day service providing social, recreational and personal supports to clients and respite, information and referral services for their caregivers. * Clients are assessed and an individualized care plan is developed * Meals and snacks are provided, and clients can also access on-site exercise and nursing services through VON. * Transportation and foot care is coordinated if needed. |
