Description and Service Details | |
Description (Service) | Provides access to in-home health and personal support services to help individuals live independently at home * Assist community members with the transition to long-term care homes when living at home is no longer possible * Can provide information about, and can link individuals to, other helpful services available in the community * Care Coordinators are health care professional specially trained to help patients investigate and organize their health care services so that patients are able to achieve the best possible state of health and well being. * Offer a single point of access to home and community services Services include: specialized nursing programs, nursing clinics, personal support services, occupational therapy, physiotherapy, social work, speech-language pathology, information and referral to community health and social services, services for children and youth, palliative care consultation, telehomecare, long-term care placement and more. |
